Beta or App Store version?

What version iOS (iOS 12 Beta or standard release iOS 11.4)?

You can turn on the Log (under Debug Settings) and Send me the Log after you produce the problem.
But you need to note the times of what you expected to happen when. Otherwise I have no idea.
You can also use a hidden feature "mark" the log by swiping across the "Savvy 20" button.

There are 4 options on Audio and that creates about 16 combinations. Combined with about 4 or more scenarios (Paired - not Paired, Streaming - not Streaming, Music or Podcast, During Call - not during call, listening to FM or Bluetooth, CarPlay no Car Play).

So that brings it to about 256 cases !!

You could post a Screen Shot of your "Sound Settings" page and describe what you want/expect it to do. And we can see if it's doing what you think it should do.

There is a bug fixed on the Speaker Option in Beta (that's been a problem for months) and I have another change coming that will only use the Speaker when it HAS to.
Because if you are listening to music or podcast and you want it V1Driver on phone speaker, EVERYTHING has to be diverted to the phone speaker, and most users do NOT want that.

If you are streaming V1Driver will always blend into your current stream. That is the least abrasive and smoothest behavior. Speaker would only be used if it cannot confirm you will hear it elsewhere.
You cannot keep music streaming to the car and V1Driver go to the speaker simultaneously.

I don't know what you mean by inconsistency of the app. You mean in muting or just audio routing?
Other times V1Driver might behave erratically is if your GPS accuracy is poor or Bluetooth signal strength is poor (there are numerous options to confirm if that is the problem or not).

Yes 11.4. Not steaming music ...using (((SiriusXM))) radio ... the gps might be a problem I noticed it said 147 feet pulling into the driveway but corrected it’s self
You have use HFP on. That's how I run it. But with that on it will always come over the Radio, no matter what, some people like that, some don't. And it will look like a "Phone Call" coming in.
If you prefer it be on the phone speaker, shut HFP Off and Speaker On. However, there is a bug in the App Store version on Speaker and it is fixed in Beta. I'll be releasing it to App Store soon.
If you just turned that on, and you're not used to that behavior it can be a little disturbing to new users. Waze and Google Maps offer the same option. Some cars do it smoother than others.

It's not unusual for GPS to start out high (e.g. pulling out of driveway). Once it does "correct itself" it should stay well below that (16ft or 33ft is typical).
If V1Driver shows that accuracy as RED that means it's not going to mute. You don't want it being RED during your normal driving. The default threshold is 100ft (above that it will turn red and stop muting).

My guess is, you just turned on HFP and are surprised by what it's doing. Because that will always try to alert over the car radio and switch your radio away from SiriusXM and then back after it's done.

HFP is defaulted Off for quite some time now.

Ok I must of accidentally turned it on ...turned it off much better now and turned off speaker ... only thing the apps doing now that it wasn’t doing before is when I use my remote start and the app connects I’m hearing the boot up sequence of the V1 over my phone/app

Sounds like you have "Sound Enable" on under Sound Settings. That emulates the V1 Brap tones during alerts. Typically only MotorCyclist use that option because they cannot hear the V1 itself.
Part that option is to also emulate the V1 Boot up Sequence when it connects. That option is Off by default.
